Report a death or incapacity

If someone you know is a My Life's Vault member and has passed away or become seriously unwell, you can let us know here. We will quietly contact the people they nominated as emergency contacts and invite them to upload supporting paperwork through a secure link.

Nothing in the vault is released until our concierge team has verified the report. Please take your time — the form only takes a few minutes.

About the member

Which details help us verify?

  • Vault reference code (from their wallet card) — fastest, single-step verification.
  • Date of birth + city / country — strong narrowing.
  • Mobile last 4 digits — disambiguates people with the same name.
  • Email address — only if you happen to know it.

Share whatever you know. If you do not have a reference code, date of birth, email, or mobile number, you can still submit the report for concierge review.

For your privacy and the member's, we won't confirm whether they are registered with us. Every report is reviewed by our concierge team before any of the member's nominated emergency contacts are notified. Nothing is released automatically.
